HOW CLINICS CAN HELP YOU WITH YOUR APPLICATION?
Staff and volunteers will help you to prepare your application and to gather the documents that Passport Canada will require. Photographers are on site for your convenience.
Not able to attend a clinic? Visit the office in Lindsay for assistance with your application.
IMPORTANT NOTE:
Passport Canada is experiencing a high volume of applications and significant delays.
The use of a clinic does not help to speed up your processing time for your application.

NEW—CHANGES TO WHO CAN USE A RENEWAL FORM—APPLIES TO ADULT APPLICATIONS ONLY
Check the date of issue. If the date your adult passport was issued is within the past 15 years and you meet the other eligibility requirements, you can apply to renew your passport.
NEW—THE CLINIC WILL NOT BE PERMITTED TO COLLECT AND SUBMIT YOUR APPLICATION.
You will be provided with an addressed envelope to send your application to the processing centre.
It is strongly recommended to send this envelope to Passport Canada using a tracking number for your parcel.
THINGS TO BRING TO PASSPORT CLINICS:
1. IDENTIFICATION:
Original birth certificate, Canadian citizenship card and/or Canadian Passport *
*(current or most recently expired passport if it is within 15 years of the issue date)
2. SECONDARY ID: REQUIRED ONLY FOR ADULT GENERAL APPLICATIONS
Photocopy of BOTH SIDES of driver’s licence or photo health card—signed by your guarantor.
The ID must be issued by federal or provincial government. It must contain: name, date of birth, sex, photo,
and signature. You may use two pieces of ID to satisfy the requirements if needed.
3. If you have a change of name, supporting documents such as a marriage certificate may be required by Passport Canada.
4. PAYMENT:
Credit card, money order, or certified cheque. *no debit or cash will be accepted as payment for a passport.
$120 Adult 5-year
$160 Adult 10-year
$57 Child (0-15 years of age)
5. REFERENCES:
Names, full mailing addresses, and phone numbers of two people NOT RELATED TO YOU who have known
you for at least 2 years.
6. GUARANTOR: ADULT GENERAL AND CHILD
Please note: A guarantor is not required when completing an Adult Simplified Renewal Application.
A guarantor is a Canadian citizen, 18 years of age or older in possession of a valid Canadian passport who
has known you for at least two (2) years. A family member or person residing at the same address as you may
be your guarantor, provided he or she meets the above requirements.
*We are unable to provide a guarantor.
7. PHOTOS
There will be a photographer on site for a fee of $20 per person. You are not required to use this service;
however, pictures are required as part of the application process. The on-site photographer will accept cash or credit card payment.
Reservations are not necessary. We hope to see you at one of our Passport Clinics.
